Book your exam

Before you pick a time, answer a few short questions about your medical history. Your answers stay in your browser — nothing is submitted until you schedule. They tell us whether you can book an exam directly or should talk with us first so your visit isn't wasted.

Preflight questions

Answer every question. If you're unsure, answer yes and we'll sort it out together.

1 Are you currently taking any prescription or over-the-counter medication?

Include anything taken regularly, even if it seems minor.

2 Have you been diagnosed with diabetes, or do you use insulin?
3 Have you ever had heart disease, a heart attack, high blood pressure, or heart surgery?
4 Have you ever had a seizure, epilepsy, a stroke, or a loss of consciousness for any reason?
5 Have you been diagnosed with or treated for a mental health condition, including ADHD, anxiety, or depression?
6 Have you ever had a DUI, a drug- or alcohol-related arrest, or been treated for substance use?
7 Have you been diagnosed with or treated for obstructive sleep apnea?
8 Do you wear glasses or contact lenses, or use a hearing aid?

Bring them with you to the exam so we can test you as you fly.

9 Has the FAA ever denied, deferred, suspended, or revoked a medical certificate for you?
10 Have you had surgery, been hospitalized, or visited a health professional in the last 3 years?
